| Purpose | Emh care and support enable learning disabled people to live a good, enjoyable and independent life through accessing support, care, housing and meaningful activities that fulfil individuals’ aspirations and needs. |
| Services | Our care and support services in Derbyshire include supported living homes, nursing homes, outreach services and supported daytime activities. We work in partnership with individuals, their families, staff, carers, and the community to ensure that people are receiving the best support for them and involving them in making their own choices and decisions. |

| Opening Times | Our supported living and nursing teams operate 24/7, according to the needs of each service. Outreach services provide hourly support, chosen by each individual. Our day services are open between 9am and 4pm, Monday to Friday. |

| Meeting Place | Our supported living services are offered in Chesterfield, Bolsover, North East Derbyshire, Derbyshire Dales and the High Peak. Our nursing homes are in Calow and near Grangewood. Our day services are offered in Tibshelf, Matlock, Bakewell and Huthwaite (Nottinghamshire). |
