| Purpose | Transition 2 (T2) is committed to supporting young adults aged 18-25 with Learning Disabilities, Severe Learning Difficulties and/or Autism as they move onto meaningful and sustainable opportunities in adulthood, helping them to release their potential and achieve genuine life outcomes in their local community. |

| Service Costs | Individual programmes of learning at Transition2 can be funded in different ways depending upon the individual circumstances of the young person. For full-time learners who attend three days per week, funding can be applied for from the Education Funding Agency via Derby City Council’s personal advisers in the Connexions team. Part-time programmes and short courses can be funded directly by the young person, using his/her Personal Budget. All funding allocated within this sector is based upon individual circumstances and the desired long-term goals of the young adult. To support this process, we encourage prospective learners and their families/carers to consider their intended end goals as soon as possible so that they may ‘start with the end in sight’. |
