Purpose

To help our customers to remain stable with their mental health, reduce the risk of relapse and going into hospital and to encourage them to use their time meaningfully. To help our customers to be as independent as they can be in all aspects of their lives and to seek out new challenges.
Services

Support through personal budgets for customers to remain independent in their own homes and through supported housing. Support ranges from budgeting, shopping, community involvement, volunteering, personal care (prompting), medication (prompting), appointments.

Eligibility Criteria Need to be referred in to our service. This can be self referral or through an organisation like Adult Social Care, YMCA, Women's Work, Derby City Council, CPN as the customer will need to have a personal budget or look to self fund.
